Overview 
On November 1, 2013, at approximately 0450-0500 hrs., Correctional Officer Verenice Rodriguez passed by female commissary and saw female inmate Johnson, Jeanette up and getting her laundry. While Johnson walked to the shower area she said "Thank You Jesus". At approximately 0520 hrs. Correctional Officer Rodriguez did her last headcount on the females in L-Tank and M-Tank and observed everyone was accounted for. On November 1, 2013 at approximately 0608 hrs. Correctional Officer Leslie Ortiz was passing out medication to females in M-Tank. Johnson did not get out of her bunk to retrieve her medication after being called by her name several times. Other females in the tank also called her name to retrieve her medication and also had no response. Correctional Officer Ortiz then entered M-Tank and once again called Johnsons name and then shook her in order to wake her up but Johnson was still unresponsive. Correctional Officer then put her hand on Johnsons back to see if she was breathing but she was not. Correctional Officer Ortiz then touched Johnsons face and it was cold to the touch. Correctional Officer Ortiz then notified Corporal Terrell to come to M-Tank and at which time Wharton E.M.S. was notified.
